Description of program:Strive was initially launched through collaborative efforts organized by Laurencrest and Youth Mental Health Services through CCH and l’Équipe psycho-sociale. In 2024, a pilot project focused on youth connected to CAS, who were experiencing complex mental health issues. This program provides families with intensive, in-home mental health services. Using a multi-disciplinary team approach, Strive offers ethically, and culturally competent services focused on reducing the severity of mental health problems impacting youth and their families.
Program duration:Case-by-case basis
What to expect:Services are individualized and flexible in delivery to address the unique needs of each client and their families. The focus is to offer services through assessment, treatment planning, counselling, and psychotherapy.
Discharge process:Upon discharge, the Strive team conducts a family meeting to discuss measurable outcomes of their involvement in the program. A final assessment is completed to visually acknowledge the successes and goals met throughout the program. A youth experience survey is distributed and encouraged to be completed.
